| Specimen: | Blood, (1) Red (no preservative) top vacutainer tube.
Draw trough levels prior to dose or at least 6 hours after last dose.
Include last dose information on request. |

| Collection: | If SST (serum separator tube) is collected, spearate serum from cells within one hour of collection. Transfer serum to a tightly capped plastic aliquot tube for transport.
Serum samples may be stored up to 2 days at room temperature or refrigerated at 2-8°C; freeze for up to 3 months. |
