Vancomycin  (VANCO (trough and peak levels); VANCO RAND (random); VANCO TR (trough only))

Synonyms:  Vancomycin, Vanco
Specimen:  Blood: (1) Red (no preservative) top vacutainer tube. Trough level- collect within 1 hour prior to dose. Peak level- collect 2 hours after infusion is complete. Random level may be collected at any time.
                        Volume:  Full primary tube
Minimum Volume:  200 ul serum
Collection:  If an SST (serum separator tube) is collected, serum must be removed for gel within 1 hour of collection. Transfer serum to a tightly capped plastic aliquot tube for transport. Refrigerate at 2-8°C up to 24 hours; freeze -20°C for 168 hours.
Rejection Criteria:  Improper labeling.
Reference Range:  Trough therapeutic: 5-15 mcg/mL Peak therapeutic: 30-40 mcg/mL
