| Specimen: | Blood; (1)Red (no preservative) top vacutainer tube. If drawn in a SST (serum separator tube) remove serum from gel within 1 hour of collection.
Label each tube with collection time.
Trough level- collect 30 minutes prior to dose.
Peak level- collect 60-90 minutes after IM dose. |

| Minimum Volume: | 200 microL serum |
| Collection: | Transfer serum to a tightly capped plastic aliquot tube for transport. Label each tube with the collection time.
Refrigerate at 2-8°C up to 6 hours; freeze at -20°C after 6 hours. |
| Rejection Criteria: | Serum received in a SST; improper labeling. |
| Reference Range: | Trough therapeutic = 0 .2-2.0 microg/mL ;Toxic- greater than 2 Peak therapeutic = 4-10 microg/mL; Toxic- greater than 10 microg/mL |
